Individuals with at least 10 years of outstanding service to patients and families as demonstrated by direct recognition from patients, families, or fellow care team members. This award is open to all non-faculty members that care for patients including social workers, technicians, patient services associates, billing, food service workers, etc.

The below must be included in a single PDF file. Individual nomination letters and letters of support must be limited to one page. For letters exceeding the one page requirement, only the first page will be considered.

1. Letter of nomination that includes reference to patient care, including meaningful contributions of patient and families for acts of caring and compassion that go beyond their call of duty. Demonstrates excellent clinical assessment skills, promotes health through education, actively engages a patient's family in care, and/or exhibits elements of safety and quality through execution of HRO skills.
2. A letter of support (can be internal or external to UM).
